Pre-operative information - Your Visit With Our Spine Doctors: What To Bring

We want to answer as many of your questions at your visit. Please prepare for this by bringing:

  • An identification card with your photograph (ex. Driver’s License, Passport, etc.).
  • Your insurance card, co-payment, billing form, or Worker’s Compensation information (Adjuster, contact information, claim number, date of injury, etc.).  Should you have any question with regards to any billing matters, please contact our billing company, DC MedBilling – (424) 250-9616.
  • Your radiology studies (actual films, not reports) pertaining to your current medical problem, (including X-rays, MRI, CT Scan, CT Myelogram, etc.)
  • The names, phone numbers, and addresses of all physicians involved in your care.  Please be sure to include primary care physicians, OB/GYNs, dentists, physical therapists, etc.
  • Prior medical records, such as Operative Reports, Procedure Reports (Epidural Steroid Injections, Selective Nerve Root Blocks, Rhizotomies, etc.), Bone Density Scans, EMG/Nerve Conduction Studies, Discograms, etc.  Please also include consultations from other physicians, particularly those from your referring physician.
  • Completed New Patient Packet (for all first-time visits).
